Job Description

GENERAL SUMMARY:

Under direction provides overall clinical coordination and guidance for the Macomb County Community Mental Health (MCCMH) system. Works with the Waiver Support Application (WSA) to process requirements necessary for the Serious Emotional Disturbance (SED) and Children’s Wellbeing Practitioner (CWP) population. Works with MCCMH departments and contracted providers provided under the Clinical Division of Macomb County Community Mental Health, with ongoing monitoring of programs. Performs related duties as assigned.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Review cases in the Waiver Support Application for eligibility, enrollment, re-enrollment, or disenrollment.

Participate in state meetings and webinars for updates on the prescreen and enrollment guidelines.

Participate in children’s administrative forum meetings, Trauma-focused cognitive behavioral therapy (TF-CBT) cohort calls, and meetings for supervisors and implementation coordinators for TF-CBT cohorts.

Review, coordinate and schedule evidence-based practice cohorts based on system need and/or grant workplan objectives/ budget narrative.

Maintain a list of all providers and staff certified in evidence-based practices throughout the network.

Participate in and coordinate necessary training as required for updates to the system and/or assessments used in children’s services, in collaboration with MDHHS and the MCCMH training center.

Assist in Behavior Treatment Plan Review Committee (BTPRC) to review and approve/disapprove cases that pose to us restrictive/intrusive interventions.

Manage grant funded projects, applications, objectives and goals, reporting, budgeting, purchases, and supplies.

Complete quarterly reporting via the EGrams system, for each grant’s respective workplan following the Michigan Department of Health and Human Services (MDHHS) guidelines.

Assist with developing and implementing data driven outcomes measurement system to address social determinants of health.

Assist with attending, conducting, and documenting care coordination meetings with the Medicaid Health Plans.

Perform weekly psychiatric admissions Follow-up after hospitalization reports, track data for each Medicaid Health Plan to provide data on the follow-up rates for each plan monthly.

Assist with data analytics and validation with the Follow-up after (FUA) HEDIS measure for the Emergency Department visit for HEDIS and Alcohol and Other Drug Dependency within 30 days stratified by race/ethnicity.

Assist with developing approaches for risk stratification and care coordination and providing and monitoring care management services for enrollees 18 and under shared by both entities (PIHP and MHP) who have significant behavioral health issues and complex physical care needs, including (but not limited to) children in foster care.

Monitor clinical policies and procedures.

Operates an automobile to perform assigned job functions.

Performs related duties as assigned.

QUALIFICATIONS:

Required Education and Experience

  • Master’s Degree in Social Work, Counseling, Psychology, Public Health or related field from an accredited college or university
  • Two (2) years or more years of experience working in the field of behavioral health and providing crisis services
  • One (1) of more years’ experience in the supervision of mental health professionals

Preferred Education and Experience

  • A minimum of three (3) years or more years of experience working in a behavioral health setting providing crisis services

Required Licenses or Certifications

  • Licensed Master Social Worker (LMSW), Limited Licensed Master of Social Work (LLMSW), Licensed Professional Counselor (LPC), Limited License Professional Counselor (LLPC), Temporary Limited Licensed Psychologist (TLLP), Limited Licensed Psychologist (LLP) or Licensed Psychologist (LP)
  • Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines
  • Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel

Preferred Licenses or Certifications

  • Certified Alcohol and Drug Counselor or Certified Advanced Alcohol and Drug Counselor (CPS, CADC or CAADC)

  • Knowledge of applicable computer applications
