Based in San Ramon, our team is growing, and we are seeking a dedicated Physical Therapist (PT) to join us. In this role, you will deliver skilled in-home therapy services, design personalized treatment plans, and work collaboratively with caregivers and clinical staff to help clients restore mobility, regain independence, and improve their overall quality of life.

Key Responsibilities

• Perform comprehensive initial evaluations and re-assessments to determine patients’ mobility, balance, and functional needs.
• Develop and implement customized treatment plans aligned with patient goals and care plans.
• Deliver gait training, range-of-motion exercises, strengthening, and fall prevention strategies.
• Educate patients and caregivers on home safety, adaptive techniques, and proper use of assistive equipment.
• Collaborate closely with PTAs, RNs, OTs, and interdisciplinary team members to ensure coordinated, high-quality care.
• Accurately complete documentation in the EMR system per Medicare and agency guidelines.
• Utilize OASIS for documentation and assessment accuracy; prior experience with OASIS is required .
• Experience with EMR systems such as Kinnser (WellSky), or similar platforms is preferred .
• Monitor patient progress, communicate changes in condition to the care team, and adjust care plans as needed.
• Supervise PTAs and aides in accordance with state and federal regulations.

Qualifications

• Current California Physical Therapist (PT) license.
• Minimum of 1 year of experience in a Home Health setting , including use of OASIS documentation.
• Proficiency with EMR systems (WellSky/Kinnser preferred).
• Strong interpersonal and communication skills.
• Ability to work independently while maintaining strong collaboration with interdisciplinary teams.
• Valid driver’s license and reliable transportation.
