The RN Case Manager will support clients and caregivers throughout our Queens, NY territory with our Preferred Home Care of NY business (a Help at Home company).

A typical day for an RN Case Manager includes:

  • Making the initial in-home evaluation visit and regularly reevaluating the patient’s nursing needs
  • Initiating the plan of care and necessary revisions
  • Preparing clinical and progress notes for each patient visit and summarization of care conferences on his/her patients in a timely manner as per Agency policy
  • Informing personnel of changes in the condition and needs of the patient
  • Counseling the patient and family/significant others in meeting nursing and related needs
  • Processing orders and notifying physician of patient needs and changes in condition
  • Completing certification/recertification orders and discharge summaries
  • Determining the amount and type of nursing and personal care support needed by each individual patient
  • Providing supervision and education to Personal Care Assistants
  • Participating in peer review and Quality Assessment and Performance Improvement as assigned
  • Meeting quality, productivity, and compliance requirements
  • Performing other related duties as assigned

Education and Experience:

  • Graduate of an accredited nursing school
  • One (1) year of clinical nursing experience, preferably in home health/home care setting
  • New York or multi-state RN licensure
  • Experience working with medically fragile clients, addressing medical, social and behavioral components of care
