Compass Healthcare Consulting & Placement is conducting a search for a RN Hospice Team Manager, Registered Nurse leader for a Hospice Program in the Bronx, NY. Qualified candidates must have NYS RN, BSN and previous Hospice RN experience within a Supervisory or Managerial role, experience and knowledge with Hospice Admissions processes, preferred.

Hospice Team Manager, Registered Nurse, RN

Manages the overall on-boarding process for patients admitted into the hospice at home program.

Responsibilities:

• Review / verify admission information and data
• Manages team for designated territories, including, RN's LPN's Social Workers, Clerical Staff, Etc.
• Ensure notification to clinician of pre-admission documentation needs
• Ensure visit schedule is generated
• Review referral information; identify resources necessary for patient admission
• Ensure Communication of face to face encounters
• Participates in orientation of new hospice staff.
• Document in clinical record all interactions with patient/family/staff.
• Serve as a liaison between Intake services and Clinical team supervisors.

Qualifications:

• Current NYS RN License
• Current Driver's License.
• Bachelor’s Degree in Nursing from an accredited school is preferred.
• A minimum of 7-10 years’ experience in nursing which includes minimum of 5 years of Hospice setting
• Management experience preferred
• Working knowledge of hospice regulations and Conditions of Participation
