Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE), dedicated to helping older adults live independently in their homes while maintaining dignity, autonomy, and a high quality of life. Our interdisciplinary team of geriatric care professionals works collaboratively in specialized Day Centers to deliver comprehensive, person-centered care.

Position Summary:
We are seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary team. In this role, you will provide therapy services to adults aged 55+ to help them maintain or regain independence in daily living activities. Your work will directly support our mission by enhancing participants’ mobility, comfort, and overall well-being—both in our centers and through in-home visits.

Key Responsibilities:

• Evaluate and treat participants to improve function in ADLs/IADLs, range of motion, strength, and pain management.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Conduct in-home assessments and provide ongoing case management as needed.
• Document care and progress in accordance with Community LIFE standards.

Qualifications:

• Education: Bachelor’s degree (or higher) in Occupational Therapy from an accredited institution.
• Licensure: Current Occupational Therapist license in Pennsylvania.
• Experience: Minimum of 2 years working with older adults.
• Other Requirements: Valid PA driver’s license and reliable vehicle.
